In order to determine your eligibility for services at SABVI, please click on the referral form for your respective area, print the form, and submit it to your eye doctor. The form, with the top half completed by the potential client and the bottom half completed by their eye doctor, needs to be returned to the address on the top of the form. Upon receipt, someone from SABVI will contact you regarding services.
